form info to collect for each iquiry (need a colab form + what we expect from each collaborator including a digital flyer - for them to post the digital flyer and tag us and for them to create a facebook event and add herbal root organics as a collaborator to the fb event (we wcan provide link to instructions for them to crate the fb event)
